Form Filling & Primary Verifications
- NYSED Online Form 1
Register your candidate profile, fill out biographical data, and submit the state license application fee ($143).
- Nursing College — Form 2F (Certification of Professional Education)
Download and print Form 2F. Take it to your Nursing College/University administration. The registrar verifies your theory/clinical training hours, signs, stamps, and seals it inside an official college envelope to mail directly to the New York State Board.
- Pakistan Nursing Council — Form 3F (Verification of Foreign Licensure)
Take Form 3F to the PNC. The PNC verifies your active local nursing registration license and dispatches the verification directly to the Board of Nursing.
- Mandatory Coursework
Complete the official online short courses for “Infection Control” and “Child Abuse Identification & Reporting” via an approved provider.
Pro-Tip: No transcripts or license verifications should be sent directly by the student. The NYSED board only accepts documents mailed directly from your College and the PNC.
Evaluation, Eligibility & ATT Generation

Monitor your Board portal status until both Form 2F (College transcripts) and Form 3F (PNC verification) are marked received and approved.
- Board Eligibility
Receive your official “Eligibility to Test” approval notification from the Board of Nursing.
- Pearson VUE Account
Create your official candidate profile on the Pearson VUE NCLEX portal.
- ATT Request
Submit the standard $200 exam fee on Pearson VUE to link your active eligibility and trigger your official Authorization to Test (ATT) letter.
Pro-Tip: Keep a close eye on your email. Once the board approves your credential files, your ATT letter from Pearson VUE has a strict validity window (usually 90 days) to schedule your exam.
